FaithSearch Teacher Training Application

Thank you for your interest in taking the FaithSearch Discovery Teacher Training. Please download and complete the application form (see below), and submit to our office (by E-mail, fax or mail) for consideration. We will contact you to discuss next steps.

For your convenience, the application form is available in two formats:

• MS Word document, which you can complete within MS Word, then print and mail or fax. Alternatively, you may save the completed application, and submit via E-mail.* You may also print the application and complete it by hand (please print neatly). < download >
• PDF form, which you can either print and complete by hand, or complete electronically by fililng in the blanks with your computer (using the free Adobe Acrobat Reader), and then printing for submission. (If you have the Adobe Acrobat application, you may also save your completed PDF and submit electronically via E-mail.*) < download >
